The people who are old will tell you how tired they feel. Not all the time, you understand. But they just seem to run out of steam and lie down in the middle of the day to recover. It never used to be like this. When they were young, they could run around at work and still have enough energy to live life to the full for the rest of the day. Now ask someone younger who’s overweight. It’s the same answer. They feel tired and listless a lot of the time. Their stamina has gone and, with increasingly less exercise in their daily program, what was a slow weight gain becomes an avalanche.
Interestingly, there’s quite a lot of research showing thin people have better stamina. They seem to maintain a better level of fitness. Physically and psychologically, heavier people find moving around more of an effort. They are more content to drop anchor on the couch, pick up a large bag of tasty snacks, and channel hop until something watchable comes along. This reflects a change in the body’s metabolism. As weight increases, hormone levels also change. When the testosterone of youth was pumping through the blood, activity levels were high. Now these men feel and act tired. Many also feel slightly depressed and, as most psychiatrists will tell you, depression is the enemy of libido.
In a recent study of men with a BMI of 30 and higher at the University of Buffalo, 40% were found to have unusually low levels of testosterone. This failure to maintain the level of testosterone rose to 50% in men diagnosed with Type 2 Diabetes. In a survey of French men, those classified as obese were more than twice as likely to report erectile dysfunction as thin men. Taken together, this could explain why millions of overweight Americans find difficulties in the bedroom.
This is not all bad news. A new study reported in the Journal of Sexual Medicine shows obese men recovered sexual function after going on a strict diet. This happened quite quickly as the weight was reduced. All the men found harder erections after losing 5% of their body weight. Interestingly, the nature of the diet made no difference. The revival of function came with the loss of weight, no matter how it was achieved. Unfortunately, the study failed to monitor the levels of testosterone. The main focus of the research was into Type 2 Diabetes and so the researchers only measured the levels of cholesterol and glucose in the blood. As a side effect, the men also reported less difficulty in urinating. The researchers speculate that just as a fall in the cholesterol levels may improve cardiovascular performance and reduce the risk of a stroke or heart attack, so a reduction in the level of inflammation in the blood vessels connected with the bladder function may ease urination.
So now the choice is yours. Cialis is an excellent drug and, in most men with reasonable levels of libido, it restores sexual activity no matter what your body weight. But if you want to feel better in yourself and improve your self-esteem, lose weight and save money on the Cialis. Would you rather be thiner and more active, or stay at your current weight and rely on a drug to keep you going?

On medical issues, there are different ways of interpreting events. Go back ten years and the question of surgery for weight loss – the so-called bariatric surgery – was viewed as an extreme response to the problem. In some senses this represented caution on the part of surgeons who wanted to ensure the procedures were safe. But it also reflected a desire to encourage people, wherever possible, to achieve weight loss through their own efforts. The concern was that, if people had proved incapable of controlling their intake of food before eating, they might continue to eat unwisely and undo the surgeon’s good work. For this reason, access to surgery was limited to cases where individuals were more than 40 BMI or more than 35 BMI if their health was adversely affected. More importantly, people had to demonstrate a genuine effort at weight loss before surgery would be authorized. This always involves dieting, an exercise routine and use of one of the anti-obesity drugs.
Today, surgery is increasingly common with thousands of patients treated every year. This is perhaps a commentary on the number of people who now qualify with a BMI of 35 or higher. It may also reflect the increasing availability of funding through health insurance. Whatever the reason, there is urgent research to determine the effectiveness of the surgery and the extent to which the patients experience an improvement in their quality of life. The picture emerging is mixed. Up to 40% of people going through surgery require follow-up treatment for complications. Significantly, real psychological problems have been identified with people requiring treatment for sometimes severe depressive disorders. These people find the physical effects of surgically restricting the amount they can eat very disruptive.
However, on the good news side, more women are able to produce children safely. Between 2003 and 2005, more than 50,000 women of child-bearing age had the surgery. Following this group has found that, when pregnant, they have fewer problems with high blood pressure, diabetes and premature birth than obese women who have not had surgery. Their babies were more consistently born at a healthy weight. Note that pregnancy is not recommended during the first year while your body is adjusting to the new physical limitations. Equally important is the need to get guidance on eating a healthy diet. It may be necessary to take vitamins and other supplements following surgery to ensure the baby is born healthy.
Surgery actually has a more dramatic effect on the body than most people realize. They are unable to eat solid food for some time, and the diet has to change to ensure they get all the nutrients required for good health. Under such circumstances, people do not need to rely on acomplia or any of the other anti-obesity drugs. Eating is significantly less pleasurable and people do not need appetite suppression. But before surgery, the use of acomplia can be appropriate as part of a real effort to lose weight naturally. The only issue to ask your doctor about is mood. As already indicated, many people become depressed after surgery. Before surgery, it is very important to stay positive and to maintain a reasonably happy mood. If not, this may not be a good indicator for them after surgery. It may also make it inappropriate to use acomplia. Always take advice and follow your doctor’s instructions before going through with surgery.

Well, let’s begin our talk with the gossip and then get serious. It seems Gabourey Sidibe who was recently nominated for an Oscar for her role in Precious, has been approached by a weight loss company. It wants to help her lose weight and, of course, by doing so enhance its own reputation. What makes the story so interesting is the aggressiveness of the approach. Here is a young woman who is obviously not unhappy with the way she looks and is successful as she is. Yet a weight loss company thinks she should want to lose weight. Now we could assume this company is altruistic. It knows being obese significantly increases the risk of diabetes, arthritis, some cancers, a stoke or heart disease. If anyone overweight can lose between 5 and 10% of their body weight, these risks are reduced or completely disappear. So why look for celebrity endorsement? The answer lies in the national statistics.
In 2009, the National Institutes of Health reported obese people in the US represented 34% of the population while the number who were merely overweight represented only 32.7%. For the record, this classifies 72 million people as obese, i.e. their BMI is 30 or higher. In fact, the rate of obesity has doubled over the last thirty years and, being dispassionate about it, this represents a major market for weight loss products and services. Billions of dollars are at stake. Against this background, the San Diego Medical Center has been running a clinical trial on POSE. This is a new approach to bariatric surgery. The increasingly common lap band procedure requires the surgeon to enter the body through the abdomen and this inevitably leaves a scar. POSE is surgery performed using an endoscope. This is a device pushed down the throat and into the stomach without any need for an incision. Once inside the stomach, there are tools operated remotely by the surgeon. This allows the stomach walls to be sutured, reducing available space by about one-third. Thus, the effect is the same as in conventional surgery. Patients begin to feel full minutes after starting to eat. Thus, for those who have vanity issues and want to achieve a “body beautiful”, this form of surgery promotes weight loss without scars.
For the purposes of the trial, only people who have a long history of weight problems are being considered. But, if the trial proves a long-term success, you will probably see this type of procedure heavily advertized for people of all weights. It’s not our policy to argue people should not have surgery. There may come a time when people prove themselves so lacking in will power to diet and exercise that physically preventing them from overeating is the only way of saving their lives. But what does concern us is the notion that surgery should become the normal response to weight problems. As a nation, do we really want to spend millions of dollars every year on surgery when the solution to the problem is a diet, physical exercise and phentermine? Just think for a moment. Phentermine is an effective appetite suppressant. Why do people using it not lose weight in the long term? Because they continue to eat massive portions of unhealthy food. People are unable to prevent themselves from overeating. What does that say about the character of Americans? We have become a nation of food junkies, so addicted to eating, we cannot stop even when we know it’s killing us.

According to Sarah Palin, the European healthcare service operates like the Canadian cull of seals. On a regular basis, men armed with clubs go out among the unhealthy and older members of society, and put down those who no longer contribute to the strength of the group. Death panels decide who shall live and die. That means, by her standards, one of the leading panels is UK’s NICE – the National Institute for Clinical Excellence. This group of so-called experts draws up guidelines to be used by the medical profession. Whenever there is good research evidence that a particular treatment is effective, it is incorporated into the national practice standards. But if a given treatment or drug is not shown effective, the medical profession and hospitals are instructed not to use them. So, many of the procedures that are standard in the US are not allowed in the UK, and many of the drugs routinely prescribed in the US are either not available in the UK or only available if the patient pays privately. Despite this, people in the UK live longer and enjoy better health than Americans.
Anyway, NICE has just launched a consultation process to decide what advice to give pregnant women about weight loss. If you want to offer an opinion, you can do so during the next month through http://guidance.nice.org.uk/PHG/Wave18/3. NICE offers the opinion that women do not need to increase the amount they eat during the first six months of pregnancy. The idea that pregnant women are “eating for two” is a myth. The most needed is an extra 200 calories a day during the last three months. That said, it is always better for women not to get pregnant if they have a BMI of 30 or more. This is not to suggest there is a high risk to the fetus. But the evidence shows there are fewer complications during pregnancy if women lose weight before getting pregnant. In general, trying to lose weight during pregnancy is not advisable. Stability and consistency of behavior are more important. That means women who routinely exercise before pregnancy should continue. Finally, women should not be misled by celebrities who slim dramatically after giving birth. It is better to lose weight gradually. Even so, there is no evidence that the quantity or quality of milk is affected in women who do crash diet after giving birth. NICE simply suggests women do what feels right for them. If they are tired, they should rest. Hopefully, they are eating a healthy diet anyway. As long as they get back to their pre-pregnancy weight at some point, this will be good for their long-term health.
There have been no tests performed in the UK or the US to determine whether it is safe for women to take phentermine hcl during pregnancy. So applying the precautionary principle, it better if you do not take this drug during pregnancy unless your doctor has specifically approved it. What can be said with certainty is that the active chemicals do enter the mother’s milk and can have an adverse effect on nursing babies. Thus, if you do want to lose weight after giving birth, do not buy phentermine online and take it while breast-feeding. You should either wean the baby or find another drug that suppresses your appetite without endangering your child.
